Patient’s Guide to Workers’ Compensation

There are several specific actions you can take that will help us with this process.

  1. Bring appropriate documentation with you including:
  • Pre-authorization or letter of referral from your employer or primary treating physician
  • Workers’ compensation insurance carrier’s information (name, address, telephone number)
  • Worker’s compensation claim number
  • Valid driver’s license or picture ID
  • Employer’s information (name, address, telephone number)

2. All patients under the age of 18 must be accompanied by a parent or legal guardian.

3. If the patient is hearing impaired or needs translation in some language other than English, Farsi or Spanish, please notify us in advance so that we may arrange for an interpreter.
